Transcript Requests

You may request your transcript via mail or fax. A form is available in our office; you may also print a copy of this form here, or you may submit the information listed below.

Transcript requests should include your full name, student ID number or Social Security number, and the address where you want the transcripts to be sent. Please include your current home mailing address and telephone number. If you fax us your request, be sure to include your daytime phone number and a credit card number with expiration date.

If you have had a name change, please provide a copy of a legal document verifying the change.

Your transcript can be sent by mail or fax. Please include the above information along with the following:

Transcript Sent by Mail (Please allow 2-3 business days to process)

1. Number of transcripts requested.
2. $3 for each transcript mailed.
3. Address of where you want the transcript(s) sent.
4. Credit card number, expiration date and v-code (last three digits found on back of card) if you want to charge payment.

Transcript Sent by Fax (Please allow 24 hours to process)

1. Number of transcripts requested.
2. $10 for each transcript faxed.
3. Fax number of where you want the transcript(s) sent.
4. Credit card number, expiration date and v-code (last three digits found on back of card) if you want to charge payment.
