Incident Reports

Incident reports are an important mechanism to record the details of an event and to prompt a review by the appropriate University officials.  Use the following table to determine the proper report.

Most forms allow for anonymous reporting; however, the ability to adequately respond to the incident may be hindered.

For emergencies, individuals should call 911 (or the country equivalent) or Campus Police at +1 (765) 677-4911.

Use for Incidents Involving:

General Incident

All non-employee incidents, including students, visitors, contractors / vendors, and volunteers.

Employee Incident

Employees, including student employees.  Includes work-related injuries, illnesses, medical incidents, exposures, property damage, accidents, environmental incidents, near misses / dangerous activities, and equipment malfunctions.

Title IX Sexual Harassment and Discrimination

Instances of sexual harassment, sexual assault, rape (including statutory), dating violence, domestic violence, stalking, or other related sexual misconduct involving members of the IWU community.

EthicsPoint

Theft, embezzlement, fraudulent activities, conflicts of interest, abuse, ethical violations, misconduct, falsification of records, and other policy violations within the IWU community.

International Incident

Crisis situations with IWU travelers working, serving, and learning outside of the United States.

Bias Incident

Any individual who experiences, witnesses, or learns of potential bias situations within the IWU community involving, but not limited to, race, ethnicity / national origin, gender / sex, disability, religion, age, veteran status, genetic information, sexual orientation, and acts of retaliation.

Academic Integrity Incident

Instances of cheating, plagiarism, falsification, collusion, and academic misconduct involving IWU-Marion students.

University Complaint

General dissatisfaction with some aspect of IWU.

Indiana Student Complaints

Ohio Student Complaints

Kentucky Student Complaint

Online Student Complaints 

Student complaints that were not able to be resolved through existing IWU processes.
