The student does not send this form to IWU. The student completes the form and submits it to his or her home church scholarship committee or youth pastor for consideration. The church must complete and send to IWU the Church Award Form for the student to be eligible for a matching scholarship. We request this form be submitted by June 30* each year.
*Please note: We request The Award Application Form be completed by June 30, annually. Church matching awards received after June 30 may not yet be on the student's financial aid award and the student could be responsible to pay the remaining balance upfront. Any church providing a scholarship for its students may participate in the Church Matching Scholarship (CMS) program. Students must maintain satisfactory academic progress, must be traditional undergraduate students, and must be enrolled for a minimum of six credit hours in order to receive a CMS grant from IWU. Matching funds cannot exceed a student's total cost.
The Church Matching Scholarship award will be rescinded if IWU does not receive the money from your church.
