Identity Verification (Dependent and Independent Students)
This form must be completed in the presence of a Notary. Submit the completed, notarized form to the Financial Aid Office via US Mail along with a copy of the unexpired valid government-issued photo identification presented to the Notary. Faxed or scanned/emailed forms are unacceptable.

Verification Documents for Independent Students
Verification forms for independent students are available in two formats. Prospective and accepted students with an active IWU email account are encouraged to complete e-forms that are electronically signed and submitted. PDF versions are available to applicants and to students who have trouble accessing or submitting the e-form. Complete only those forms the Financial Aid Office has indicated you need to complete.
