If you would like to place an order for a Survival Kit for your student, or make a donation to help other students receive a Kit, this is the form for you.

How to place your order:

• Order one for your student
  Order your kits by April 9, 2010, one kit for $15.00. (Remember, we need to order the Survival Kit items, so your cooperation in meeting this deadline will GREATLY assist in our planning and ordering). Students will receive notice to pick up their Survival Kit on April 20^th.
  
  
• Order additional kits for other students
  You may choose to send a donation of $50.00 to supply kits for missionary kids, international students or students who would not otherwise receive one. This gift is tax deductible.*
